How does your place handle bad calls? Like really bad?

Every once in a while we’ll get a lewd caller or someone being abusive or racist… just really nasty calls that can shake someone to the core. Fortunately we are allowed to hang up when it happens as long as we report why we hung up on someone. But nothing really happens after. How does your company handle these calls? Do you file incident reports with HR or just go on with your day?
